When authorized, licensed and available, you will be able to see 12 months of filled prescription medication history for a selected patient. The drug history download, also referred to as the Medical History for Reconciliation (MHR, contains medication history for the selected patient and can be accessed after you receive consent from the patient. It is your responsibility to obtain the patient`s consent prior to accessing the MHR. The MHR is not a complete list of all medications for patients and note that IT MAY CONTAIN ERRORS or OMISSIONS. It is the responsibility of any treating provider or other health care provider or facility to verify prescription medication history information through other means with each patient and/or the patient representatives before such information is relied upon or utilized in diagnosing or treating the patient. By using this data, you agree you understand the limitations of the MHR data and shall release and holds harmless the application developer from any liability, cause of action, or claim related to the completeness or lack thereof of the medication history information. These limitations also apply to any prescription benefit information provided in the DAW Applications.
To View Downloaded Medication History:
- Log in to ScriptSure at https://app.scriptsure.com/login
- Locate and select the patient from the search results
- Upon selecting the patient, the system will automatically show the patient's chart and the downloaded drug history will appear at the bottom of the screen:
Note: As the data is provided by insurance companies, you will only see data for patient's that have insurance. Also, in order to obtain the drug history download, the Patient Consents to Downloaded Med History needs to be marked YES. If it is not marked yes, you can modify this on the patient demographics screen by clicking EDIT. The goal of this setting is to ensure you have asked your patient if you have their approval to obtain this data. See more on demographics here: Add New Patient
